The Victoria Proof of Age card is used to display your exact age. It is used to prove age over 18 to legally purchase alcohol. This card is recognized throughout Australia.
You can apply for a Victoria Proof of Age card if you are over 17 and 11 months old, however, you must wait until you are 18 to use it. Application fee is $ 10.

Change name
If you have changed your name, through marriage or otherwise, you must also provide documentation as evidence of this change.
Get your documents signed
Bring your completed form and proof of identification to someone who knows you for at least 12 months and is listed on the Australian electoral roll.
The authorized person will check the two documents you provided as proof of identity and then complete and sign the 'Authorized Declaration'.
